The excellent web site of Connecticut's daily Hartford Courant carries a well-thought-out rate card for banner ads and for ancillary services like email, forms, design, scanning and linking. I don't know what their circulation (IPs/month) is, but they charge $500 for setup, $150 per month for the banner plus $100 per month for a link, all with a three month minimum.
